Hallucination

noun/hə·loo·si·nay·shən/

When an AI model states something that sounds plausible but is wrong or made up. The safeguards are grounding answers in your own sources and having a person check facts before anything is used.

In practice

Hallucination: what it means for your business

Hallucinations are why AI output needs checking. They are most likely when a model is asked for specific facts, figures, quotes or sources it does not have. The practical safeguards are giving Claude the source material to work from, asking it to cite where answers came from, and having a person check facts before anything is sent or published.

How do you stop AI hallucinations?

You cannot remove them entirely, but you can reduce them: ground answers in your own documents, ask for sources, tell the model it may say it does not know, and keep a person reviewing anything factual before it is used.
